Overactive Bladder Syndrome

Overactive bladder (OAB) syndrome is a common condition affecting both men and women.

The hallmark symptom is a sudden, compelling urge to urinate that may be difficult to defer. This often occurs alongside urinary frequency and waking overnight to pass urine (nocturia). In more severe cases, OAB may also lead to episodes of urinary leakage (urge incontinence).

Diagnosis

Diagnosing OAB is primarily based on the symptoms you describe, including urgency, frequency, and nocturia. Validated questionnaires and bladder diaries are valuable tools to measure and track these symptoms accurately.


A thorough clinical examination will be carried out to rule out other potential causes. Initial investigations typically include urine analysis to check for infection and urinary flow testing. In selected cases, imaging such as ultrasound or cystoscopy may be recommended.


For more complex or persistent symptoms, one of the London Urology Collective specialists may arrange advanced bladder function assessment using urodynamic studies (pressure–flow studies), which provide detailed information about bladder activity.

Management Options

There are several effective treatments available to help control OAB symptoms and improve quality of life.

Conservative (First-Line) Approaches

Many patients experience improvement through lifestyle and behavioural measures, including:

  • Bladder training to improve bladder control

  • Optimising fluid intake to reduce bladder irritation

  • Pelvic floor muscle exercises, which can enhance sphincter support and improve continence

These strategies are often the first step in managing OAB.

Medications (Second-Line)

If conservative therapies are insufficient, your specialist may prescribe medication. Options include:

  • Anticholinergics

  • Beta-3 agonists


These medications help relax the bladder muscle and reduce urgency, frequency, and leakage episodes. Your specialist will recommend the most appropriate treatment based on your symptoms and medical history.

Advanced Therapies (Third-Line)

For individuals who do not respond adequately to conservative treatments or medication, additional options are available:

  • Neuromodulation – gentle electrical stimulation of specific nerves to improve bladder function

  • Botulinum toxin (Botox) injections into the bladder muscle to reduce overactivity and urgency.

Both treatments can be very effective for persistent OAB.

Surgical Options (Fourth-Line)

In cases of severe, end-stage OAB that does not respond to other therapies, major reconstructive surgery such as bladder augmentation may be considered. This is reserved for a very small number of patients with complex bladder dysfunction.
